New Patient Forms

On the day of your first appointment, you will be seeing the dentist for a full check-up and X-rays and the hygienist for a dental cleaning. Your appointment will be for approximately 60 - 90 minutes.

You will be asked to come approximately 15 minutes before your appointment time to fill out your new patient forms and medical history or you can download them from our website and fill them out ahead of time.
